1. CPA की सदस्यता के लिए रजिस्ट्रेशन फॉर्म भरने से पहले, कृपया छत्तीसगढ़ फार्मासिस्ट एसोसिएशन (CPA) के संबंध में नीचे दिए गए विनियमों को ध्यान से पढ़ें।
2. ऑनलाइन रजिस्ट्रेशन फॉर्म से संबंधित सभी महत्वपूर्ण सूचना पंजीकृत ईमेल आईडी पर भेजे जाएंगे। फार्मासिस्टों को सलाह दी जाती है कि वे अपनी सही ईमेल आईडी / मोबाइल नंबर (उपयोग में) दें।
3. रजिस्ट्रेशन हेतु सदस्यता शुल्क केवल ऑनलाइन माध्यम (डेबिट कार्ड, नेट बैंकिंग अथवा UPI पेमेंट) से ही स्वीकार किये जायेंगे।
4. हाल ही में खींचा हुआ पासपोर्ट आकार का रंगीन फोटोग्राफ (फोटो केवल "JPEG" प्रारूप में स्कैन करके अपलोड करें । स्कैन किए गए फोटोग्राफ का फ़ाइल साइज 200 KB, चौड़ाई 250 Pixs और ऊंचाई 300 Pixs से अधिक नहीं होना चाहिए)।
5. अस्पष्ट अपलोड की गई डिजिटल फोटोग्राफ वाले फार्मासिस्ट एवं रजिस्ट्रेशन शुल्क जमा नहीं करने पर रजिस्ट्रेशन फॉर्म को अस्वीकार कर दिया जाएगा।
6. CPA की सदस्यता के लिए किसी भी अतिरिक्त जानकारी या पूछताछ के बारे में ईमेल: cparaipurag81@gmail.com भेज सकता है। किसी भी ईमेल पत्राचार में स्वचालित रूप से प्रदान की गई ऑनलाइन रजिस्ट्रेशन आईडी का उल्लेख करना आवश्यक है।

The procedure for submitting the online membership registration form is as follows :
1. Before filling out the registration form for CPA membership, please carefully read the regulations regarding the Chhattisgarh Pharmacists Association (CPA) provided below.
2. All important information related to the online registration form will be sent to the registered email ID. Pharmacists are advised to provide their correct email ID and mobile number (currently in use).
3. The membership fee for registration will be accepted only through online modes (Debit Card, Net Banking, or UPI Payment).
4. A recently taken passport-sized color photograph is required (Please scan and upload the photo only in "JPEG" format. The file size of the scanned photograph must not exceed 200 KB, with a width of 250 pixels and a height of 300 pixels).
5. Registration forms submitted by pharmacists with unclear digital photographs, or those submitted without payment of the registration fee, will be rejected.
6. For any additional information or inquiries regarding CPA membership, you may send an email to: cparaipurag81@gmail.com. It is mandatory to mention the automatically generated Online Registration ID in all email correspondence.
7. Pharmacists are required to submit the CPA membership registration form electronically via the online portal only; hard copies of the registration form will not be accepted.
8. Upon the successful submission of the registration form details, an automatically generated Application Registration Number and a confirmation message will appear on your screen. You will also receive an email containing an Application ID code at the email address you provided in the registration form.
9. Please use this Online Application ID for all future correspondence.
10. Pharmacists must provide accurate information while filling out the online registration form; otherwise, if any errors are detected in the provided information, the pharmacist will be held solely responsible, and the registration form will be cancelled. 11. Finally, for future reference, please take a printout of the completed online registration form.
12. The membership fee for the Chhattisgarh Pharmacist Association has been fixed at ₹300/- for a period of one year (from April 1st to March 31st). It is mandatory to pay the membership fee annually to maintain membership in the Association. Failure to pay the annual membership fee will result in the cancellation of the Association membership.

1. हमारी नीति यह है कि एक बार जब कोई फ़ार्मासिस्ट सदस्यता के लिए छत्तीसगढ़ फार्मासिस्ट एसोसिएशन की वेबसाइट (www.cparaipur.co.in) के माध्यम से रजिस्टर्ड हो जाता है, तो ट्रांज़ैक्शन शुल्क अंतिम माना जाता है, और रजिस्टर्ड व्यक्ति को उनसे लिए गए रजिस्ट्रेशन शुल्क की वापसी (रिफ़ंड) की माँग करने की अनुमति नहीं होगी।
2. यह फिर से दोहराया जाता है कि किसी भी कारण से, किसी भी रूप में (नकद या वस्तु के रूप में) कोई रिफ़ंड नहीं दिया जाएगा, और सभी ट्रांज़ैक्शन अंतिम माने जाएँगे।
3. शुल्क का कोई रिफ़ंड नहीं दिया जाएगा, भले ही रजिस्टर्ड व्यक्ति किसी भी कारण से संस्था में शामिल न हो पाए और अपना रजिस्ट्रेशन हमें वापस कर दे या किसी और को ट्रांसफ़र कर दे।
4. वेबसाइट पर ऑनलाइन माध्यम से फ़ार्मासिस्ट द्वारा एसोसिएशन की सदस्यता केवल 1 वर्ष के लिए भुगतान किया गया शुल्क अंतिम माना जाएगा। किसी भी स्थिति में शुल्क वापस नहीं किया जाएगा।
5. यदि रजिस्ट्रेशन फॉर्म भरते समय फार्मासिस्ट गलती से रजिस्ट्रेशन शुल्क का भुगतान एक से अधिक बार कर देता है, तो वह अतिरिक्त भुगतान 48 से 72 घंटों के भीतर उसके खाते में वापस (रिफ़ंड) कर दिया जाएगा।
6. गलतफहमी और विवादों से बचने के लिए, इस 'कोई रिफ़ंड नहीं' (No Refund) नीति की जानकारी उपयोगकर्ता को इसके द्वारा दी जाती है।
7. “एक बार पेमेंट हो जाने पर जो किसी भी कारण से नॉन-रिफंडेबल या नॉन-कैंसलेबल होते हैं”|
8. “किसी भी पेमेंट (डुप्लिकेट पेमेंट सहित) के लिए ट्रांज़ैक्शन फ़ीस कार्डहोल्डर को देनी होगी”|
9. “किसी भी रिफ़ंड/रिवर्सल/चार्जबैक/किसी भी अन्य कारण और यहाँ तक कि डुप्लिकेट पेमेंट के लिए भी ट्रांज़ैक्शन फ़ीस चार्ज किसी भी हालत में रिफ़ंड/रिवर्स नहीं किया जाएगा ।”

Membership Fees, Cancellations, and Refunds Terms & Conditions and Privacy Policy :
1. Our policy states that once a pharmacist has registered for membership via the Chhattisgarh Pharmacist Association website (www.cparaipur.co.in), the transaction is deemed final; the registered individual shall not be permitted to request a refund of the registration fee paid.
2. It is reiterated that, for any reason whatsoever, no refunds—in any form (whether cash or in kind)—will be issued, and all transactions shall be considered final.
3. No refund of fees will be granted, even if the registered individual is unable to join the organization for any reason, or chooses to surrender their registration to us, or transfers it to another party.
4. Regarding Association membership obtained online through the website, the fee paid by a pharmacist for a one-year term is considered final. Under no circumstances will this fee be refunded.
5. If, while filling out the registration form, a pharmacist inadvertently pays the registration fee more than once, the excess payment will be refunded to their account within 48 to 72 hours.
6. To prevent misunderstandings and disputes, the user is hereby formally notified of this 'No Refund' policy.
7. “Once payment made which are non-refundable or non-cancellable for any reason”
8. “Transaction fees charged would be borne by cardholder for any payment (including Duplicate payments)”
9. “Transaction fee charges would not be refunded/ reversed under any circumstances for any refund/ reversal /chargeback /any other reasons and even duplicate payments.”
